Job Summary

Carmel Terrace, a Faith-Based, Nonprofit, Assisted Living facility sponsored by the Carmelite Sisters, seeks a skilled Wellness Nurse to join our team. As a key member of our healthcare staff, you will provide exceptional resident care and work collaboratively with our interdisciplinary team.

Responsibilities
  • Implement and coordinate resident care in collaboration with resident, physician, and nursing staff.
  • Document nursing care, including care/service plans, residents' overall condition, and behavior.
  • Provide functional direction and assistance to LPNs and CNAs.
  • Complete new staff orientation as assigned.
  • Direct resident care, including making rounds, answering call lights, administering/observing medications.
  • Complete monthly/quarterly assessments and documentation.
  • Attend care/service plan meetings.
  • Communicate effectively with residents, families, visitors, and members of the healthcare team.
  • Participate in screening potential admissions.
Requirements
  • MA LPN - Licensed Practical Nurse License.
  • 1 year of Long-Term Care experience required.
Salary and Benefits
