OVERVIEW:

As a Behavioral Care Coordinator at our esteemed organization, you will be instrumental in overseeing and facilitating mental health services within skilled nursing environments. Your collaboration with facility personnel, healthcare experts, and our dedicated team will be pivotal in delivering exceptional behavioral healthcare tailored to the varied needs of residents. Your dedication to patient welfare and proficiency in behavioral health will significantly improve the overall quality of care provided.

KEY RESPONSIBILITIES:

  • Engage in clinical training, case discussions, and regular in-person meetings with the facility's clinical team.
  • Conduct on-site visits with nursing facility staff to observe and document resident behaviors in accordance with CMS guidelines.
  • Aid in the execution of behavioral intervention strategies aimed at managing difficult behaviors and promoting positive coping skills among residents.
  • Work closely with facility teams to integrate behavioral health services seamlessly into individualized care plans.
  • Ensure precise documentation in compliance with regulatory standards, company policies, and best practices.
  • Consult with healthcare professionals for referrals to specialized behavioral health services as necessary.
  • Participate in company initiatives aimed at enhancing the delivery of behavioral health services in skilled nursing facilities.
  • Uphold compliance with healthcare regulations and standards.
  • Stay informed on training and certifications pertinent to the role.
  • Follow reporting standards and company compliance protocols.
  • Carry out additional responsibilities as assigned by your supervisor.

REQUIRED COMPETENCIES:

TRAVEL REQUIREMENTS:

Travel to designated nursing facilities is necessary.

ESSENTIAL REQUIREMENTS:

  • Capability to perform essential functions with or without accommodations.
  • Compliance with facility policies, infection control measures, and safety protocols.
  • Current TB screening and COVID-19 vaccination as required.
  • Reliable transportation for travel to nursing facilities.

EDUCATIONAL BACKGROUND & EXPERIENCE:

  • High School Diploma or Equivalent is required.
  • An associate or bachelor's degree in psychology, social work, or a related field is preferred.
  • A minimum of 2 years of experience in a behavioral healthcare environment is required.

CERTIFICATIONS AND LICENSES:

  • CPR & First Aid Certification is mandatory.
  • Non-violent Crisis Prevention (CPI) Certification is preferred.

SUPERVISORY DUTIES:

  • None.

WORK ENVIRONMENT:

This position primarily operates within nursing facilities, with some administrative responsibilities in an office setting. The role necessitates interaction with various individuals, making strong communication skills essential. Efficient management of electronic health records systems and the use of technology for patient care are critical components of this position.

WORKING CONDITIONS:

  • Requires remaining stationary for extended periods.
  • Involves frequent walking within the office and nursing facility.
  • Utilization of technology and office equipment.
  • Ability to lift, bend, and engage with residents.
  • Effective communication and auditory perception skills are necessary.
  • Adherence to infection control protocols is required.
