Centralized Nurse Care Coordinator

We are seeking a highly skilled and compassionate Centralized Nurse Care Coordinator to join our team at CHAS Health. As a key member of our care coordination team, you will play a vital role in improving the overall health of our communities by supporting patients with complex chronic conditions.
Key Responsibilities- Analyze patient registries and reports to identify individuals in need of additional support and develop effective care plans.
- Conduct thorough patient chart reviews and interviews to identify clinical and social determinants of health barriers.
- Collaborate with patients, care teams, and outside entities to coordinate care and ensure seamless transitions.
- Support patient care teams in creating and implementing care plans, with a focus on patient engagement and adherence to prescribed therapies.
- Assist with tracking and presenting outcomes for assigned populations to leadership.
- Bachelor's degree in nursing from an accredited school of nursing or equivalent experience.
- Registered Nurse (RN) license in the State of Washington required upon hire and eligible to obtain RN license in Idaho.
- Community health setting experience preferred.
- Experience with Chronic Disease Management and electronic health records preferred.
- Competitive compensation package, including a base salary range of $79,100 - $119,646 USD annually.
