RN Care Navigator

The RN Care Navigator is a critical member of our healthcare team, responsible for managing patient populations with chronic illness and/or multiple co-morbidities. This individual works in coordination with multiple PCPs and serves as the central link to an identified population of patients.
Key Responsibilities- Manage patient populations with chronic illness and/or multiple co-morbidities
- Work in coordination with multiple PCPs
- Serve as the central link to an identified population of patients
- Graduation from an accredited school of Nursing
- Washington Registered Nurse License upon hire
- National Provider BLS - American Heart Association upon hire
- 2 years - Work experience in an RN role
- Bachelor's Degree - Nursing
- Certification of diabetes education (CDE) upon hire
- Experience managing complex patients in an ambulatory setting
- Medical Home Model experience
