Job Summary

Family First is seeking a highly skilled and compassionate Home Care Coordinator to join our team. As a key member of our Home Care department, you will be responsible for providing mentorship, training, and support to in-home CarePros, while also delivering exceptional customer service and professional care to clients.

Key Responsibilities:

  • Supervise and train direct care staff, including hiring, training, and evaluation
  • Conduct regular supervisory visits and provide coaching and counseling as needed
  • Develop and implement customized care plans for each client, ensuring optimal care and support
  • Manage client and CarePro communication, addressing issues and concerns in a timely and professional manner
  • Collaborate with the Home Care department to ensure efficient staffing and scheduling

Requirements:

  • Minimum two years supervisory experience required
  • Current Certified Nursing Assistant or Home Care Aide Certification required
  • Nurse Delegation experience preferred
  • Strong leadership skills and ability to work independently
  • Excellent communication and interpersonal skills
