Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our team at Delta-T Group Inc. in Washington, DC. As a Case Manager, you will play a critical role in supporting the well-being of youth and families by providing expert knowledge and guidance to help sustain program requirements.

Key Responsibilities
  • Takes a position of leadership in supporting program structure and all youth within the program.
  • Demonstrates initiative in supporting and empowering the youth and families.
  • Ensures client files, records, monthly reviews, and Individual Service Plans are accurate, updated, and secure.
  • Participates in live supervision with clients and their families.
  • Meets regularly with the Program Manager for supervision.
  • Demonstrates engagement and leadership in all weekly staff meetings.
  • Assists clients with discharge and aftercare planning as they transition into their next step following the program.
  • Provides the Program Manager with timely and accurate documentation of all services provided, including client progress reports and reports of problems that require immediate attention.
  • Oversees and reviews client assessments, provides case management services, and oversees the development of individual treatment plans for clients and families.
  • Other duties as assigned.
Requirements
  • Bachelor's or Master's Degree in Social Work.
  • 1-5 years of experience in the Human Services field, preferably working with adolescents and/or young adults.
  • Expert knowledge of issues surrounding homelessness.
  • Demonstrated ability to work with community leaders to raise funding for programs and skills to conceptualize and effectively lead complex projects.
