Job Title: Travel Nurse RN Case Manager
Job Summary:
Planet Healthcare is seeking a skilled Travel Nurse RN Case Manager to join our team. As a Travel Nurse RN Case Manager, you will be responsible for providing high-quality patient care and case management services to patients in a variety of settings.
Responsibilities:
- Provide patient care and case management services to patients in a variety of settings, including hospitals, clinics, and nursing homes.
- Assess patient needs and develop individualized care plans to meet those needs.
- Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Document patient progress and outcomes in accordance with regulatory requirements.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements:
- Current RN licensure in the state of practice.
- Minimum of 2 years of experience as a Registered Nurse, preferably in a case management role.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Basic computer skills and proficiency in electronic medical records.
