Job Summary: The Licensed Practical Nurse (LPN) collaborates with healthcare professionals to deliver both direct and indirect patient care in an outpatient environment.

The LPN is responsible for providing nursing services to designated patients under the guidance of a physician or Registered Nurse (RN), accepting delegated tasks to fulfill patient needs.

This role adheres to the policies and procedures of Monument Health, professional standards, and complies with the South Dakota Nurse Practice Act, along with other relevant federal, state, and local regulatory agencies.

Essential Functions:
- Prepare examination and treatment areas according to clinic standards.
- Engage in pre-visit planning and ensure readiness for patient care.
- Conduct patient rooming procedures, including vital signs and data collection specific to the clinic.
- Administer and document medications accurately, following best practices and clinic policies.
- Perform nursing procedures and treatments within the defined Scope of Responsibilities according to nursing division and clinic standards.

- Demonstrate proficiency in performing and documenting assessments, applying clinical interventions, evaluating outcomes, and modifying care plans in collaboration with RN/provider.
- Distribute preapproved patient education materials and reinforce health teachings provided by RN/provider, directing patient inquiries to RN/provider.
- Conduct independent nurse visits as permitted by scope and departmental policy with appropriate supervision.
- Assist in managing inbox and complete patient referrals/prior authorizations per policy and scope.
- Participate in onboarding and mentoring of clinical staff as directed.
- Contribute positively to departmental development through participation in meetings, committees, and continuing education/training.
- Ensure documentation in patient records is accurate, clear, and concise within the outlined Scope of Responsibilities.
- Perform all other duties as assigned.

Additional Requirements:
Required:
- Education: Completion of a nursing education program approved by a board of nursing.
- Certification: Licensed Practical Nurse (LPN) - South Dakota Board of Nursing.
Preferred:
- Experience: 1+ years of Healthcare Experience.

Physical Requirements:
This position involves very heavy work, requiring the exertion of over 25 pounds of force constantly, more than 50 pounds frequently, and more than 100 pounds occasionally. There may be exposure to radiation and frequent contact with contaminated needles and infectious body substances.

Ability to work designated shift lengths (including 8 and 12 hours), which may include night shifts or occasional irregular hours is required.
