About the Role

Fellowship Square is seeking a dedicated and compassionate Registered Nurse Case Manager to join our Home Health Team.

Key Responsibilities
  • Administer skilled nursing care to patients on an intermittent basis in their place of residence.
  • Develop and implement individualized plans of care in accordance with physician orders and Agency policies.
  • Conduct initial evaluations and regular reevaluations to assess patient needs and make necessary revisions to the plan of care.
  • Provide preventive and rehabilitative nursing procedures as needed.
  • Document patient progress and communicate with healthcare team members to ensure seamless care.
  • Counsel patients and their families on meeting nursing and related needs.
  • Process orders and notify physicians of patient needs and changes in condition.
  • Supervise and teach other nursing personnel as needed.
Requirements
  • Graduate of an approved school of professional nursing and currently licensed in the state of Arizona.
  • Two years' nursing experience in Home Health, preferably with case management experience.
  • Ability to exercise initiative and independent judgment.
