Job Title: Nurse Care Coordinator

Key Responsibilities:

  • Collaborate with patients, families, and healthcare teams to develop and implement individualized care plans.
  • Conduct thorough assessments of patients' needs and develop strategies to address gaps in care.
  • Coordinate with healthcare providers to ensure timely and effective transitions of care.
  • Provide education and support to patients and families to promote self-management and independence.
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed.

Requirements:

  • Associate or bachelor's degree in nursing with LPN/LVN license.
  • 1-3 years of experience in nursing or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize multiple tasks.
