About the Role

We are seeking a highly skilled Clinical Social Worker to join our Oncology Service Department at Elliot Health Systems. As a key member of our team, you will play a vital role in providing exceptional patient care and support.

Key Responsibilities
  • Collaborate with the Multidisciplinary Team: Work closely with our primary nurse and the multidisciplinary team to conduct ongoing clinical social work assessments of each patient.
  • Identify Community Resources: Identify relevant community resources and link them to patients as appropriate, ensuring they receive the support they need.
  • Provide Supportive Counseling: Offer supportive counseling and crisis intervention to patients and their families as needed.
  • Coordinate Care: Coordinate with key personnel at community health facilities to ensure patient needs are met.
  • Homecare Referrals: Coordinate homecare referrals, acute rehab, and nursing home referrals as defined.
  • Community Liaison: Serve as a resource and liaison to community social service agencies relative to identified patient and family needs.
Requirements
  • Master of Science in Social Work: Required
  • Experience in Oncology: One to three years of experience in working with Oncology patients and cancer survivors, preferred.
  • Excellent Clinical Documentation: Excellent clinical documentation and successful usage of EMR systems, preferred.
