We are seeking a skilled LPN to join our Primary Care Network team in an Internal Medicine practice providing outpatient services for adults 18 years and older.
Responsibilities- Provide direct patient care, including administration of medication and vaccines, minor laboratory testing, and communication with patients and providers using EMR.
- Work collaboratively with a team of Ambulatory Techs, RNs, Physicians, and APPs to deliver high-quality patient care.
- Participate in preventative health maintenance, chronic, and acute care needs.
- Diploma from an accredited LPN Nursing Program.
- Active New York State LPN license in good standing.
