Position Overview: We are looking for a dedicated Registered Nurse Care Coordinator who is committed to making a meaningful impact in the lives of individuals in rural communities. This role is essential in managing patient care and ensuring that our members receive the necessary support for their health journeys.

Key Responsibilities:

  • Lead comprehensive care management for patients with chronic conditions and facilitate annual preventive screenings.
  • Provide both in-person and virtual care, demonstrating adaptability and comfort with various modalities.
  • Document and maintain accurate patient records, including vital signs, medical history, and treatment interactions.
  • Oversee the prescription process, including medication reviews and prior authorizations.
  • Ensure compliance with quality measure requirements, including HEDIS and STARS metrics.
  • Manage risk adjustment coding for a designated panel of members.
  • Prepare patient rooms and necessary equipment for mobile clinic visits.

Qualifications:

  • Registered Nurse degree from an accredited nursing program.
  • A strong passion for providing care in rural settings.
  • Experience with electronic health record (EHR) systems is advantageous.
  • Ability to deliver high-quality clinical care while maintaining cost-effectiveness.
