We are seeking a highly skilled and experienced Clinical Coordinator to join our team at DaVita Inc. As a Clinical Coordinator, you will play a vital role in ensuring the delivery of high-quality patient care in a hospital setting.

Key Responsibilities
  • Coordinate patient care and services, including trend lab values, vital signs, weight, and outcome management.
  • Ensure the highest level of care is delivered to patients by ensuring their safety, comfort, and well-being.
  • Train and mentor staff to deliver exceptional patient care.
  • Collaborate with interdisciplinary teams to develop and implement patient care plans.
  • Monitor and report on patient outcomes and quality metrics.
Requirements
  • Current Registered Nurse (RN) license in the state of practice.
  • At least 18 months of registered nursing experience.
  • Dialysis Charge RN readiness approval by clinical Nurse Manager or RN-licensed FA/GFA required.
  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) preferred.
  • Intensive Care Unit (ICU), Critical Care Unit (CCU), Emergency Room (ER), or Medical Surgical (Med Surg) experience is preferred.
