Job Summary:The Independent Living Case Manager will assist consumers with a variety of services and training to enable them to live independently in their community. This role requires a strong understanding of independent living philosophy and the ability to relate and empathize with people with disabilities.
Key Responsibilities:- Develop Supported Living Service Plans
- Maintain communication with Personal Care Attendants (PCAs) and other service providers
- Assist consumers in hiring, training, and supervising PCAs
- Help consumers obtain housing and access community resources
- Encourage and assist consumers in developing relationships with community members
- Maintain confidential records and follow program guidelines
- Train consumers on human rights and how to obtain assistance on human rights violations
- Assist consumers with Transitional Assistance services through the Money Follows the Person (MFP) and Acquired Brain Injury (ABI) waivers
- Bachelor's degree and/or experience serving people with disabilities
- Familiarity with community services and independent living philosophy
- Strong communication, organization, and writing skills
- Training and supervisory experience
- Significant experience in PCA services, case management, and disability service delivery systems
This job operates in a professional office environment and requires frequent travel. A valid driver's license and reliable transportation are necessary.
