Case Manager

FamilyAid is a leading provider of solutions to family homelessness, empowering parents and caregivers to secure and sustain housing and build foundations for their children's futures.
We prevent family homelessness whenever possible, reduce harm if homelessness can't be avoided, and provide wraparound supports that foster long-term stability. Our two-generation educational programs, housing search and placement support, and connections to vital resources help children and parents achieve self-sufficiency.
As a Case Manager, you will work with homeless families at our Family Navigation Center or shelters, utilizing our approved two-generation, trauma-informed care model and service plan to ensure each family member accesses support and resources for daily living and long-term stability.
Key Responsibilities:
- Develop and implement individualized service plans for families
- Provide trauma-informed care and support to families
- Connect families with community resources and services
- Monitor and evaluate program effectiveness
Requirements:
- Bachelor's degree in social work or related field
- Professional experience in social services, case management, or social work
- Bilingual in English and Haitian Creole or Spanish
- Ability to work in office and field-based settings with a diverse homeless population
