Senior Care Case Manager

Job Summary:
This position is an opportunity to work as part of a vibrant multidisciplinary team at Boston Health Care for the Homeless Program (BHCHP). The Case Manager will work closely with other care team members to engage senior homeless individuals in primary care and services addressing social determinants of health needs.
Key Responsibilities:
- Work with patients in various settings, including outreach and clinics, to reduce barriers to healthcare and treatment.
- Connect patients with community resources and social services.
- Conduct timely assessments and document progress.
- Assist with arranging appointments and accompanying patients to services.
- Triage and troubleshoot care needs issues.
- Participate in team case conferences and advocate for patients' needs.
Requirements and Qualifications:
- A bachelor's degree in social work, human services, or a related field.
- Excellent customer service skills and ability to communicate professionally.
- Efficient, organized, and detail-oriented.
- Self-directed and able to work independently and as part of a team.
- Motivational Interviewing and Cultural Competency training a plus.
