Key Responsibilities:

  • Provide individualized care to patients with complex medical and behavioral needs
  • Collaborate with multidisciplinary teams to develop and implement patient care plans
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective treatment plans
  • Administer medications and treatments as prescribed by physicians
  • Document patient progress and maintain accurate records

Requirements:

  • One year of post-education nursing experience preferred
  • Experience working with individuals with cognitive and/or behavioral challenges or medically complex conditions
  • Proficient computer skills and experience with data collection and analysis platforms
  • Strong organizational and communication skills
  • Ability to work independently and prioritize multiple tasks simultaneously
