About the Position:
This part-time (20 hours) Nurse Case Manager position is located in Leominster, Fitchburg, Westminster, Leicester, and Rutland areas. As a Nurse Case Manager, you will play a critical role in coordinating client discharges, conducting intake assessments, and ensuring accurate medication management.
The ideal candidate will possess a strong understanding of basic nursing concepts, including pharmacology, and thrive in a fast-paced environment. Excellent communication skills, organizational abilities, and self-motivation are essential for success in this position.

Key Responsibilities:- Coordinate client discharges and conduct intake assessments to ensure seamless care transitions.
- Audit in-home medications and medication administration records to guarantee accuracy and safety.
- Manage new medication orders and provide training to staff on new diagnoses.
- Valid state nursing license as an RN.
- Minimum 1 year of relevant experience.
- Self-motivated and detail-oriented with ability to multi-task.
