About the Role

We are seeking a highly skilled Care Coordinator to join our team at South Middlesex Opportunites. As a key member of our behavioral health community partner program, you will play a vital role in providing comprehensive care coordination services to assigned enrollees.

The estimated salary for this position is $60,000 - $75,000 per year, depending on experience and qualifications. This compensation package includes comprehensive benefits, paid time off, and opportunities for professional growth and development.

Key Responsibilities

  • Develop and implement personalized treatment plans for enrollees, incorporating their unique needs and goals.
  • Conduct regular outreach and engagement activities to promote enrollment in our behavioral health community partner program.
  • Collaborate with clinical care managers, RNs, and other care team members to ensure seamless care coordination and communication.
  • Provide health and wellness coaching to enrollees, empowering them to make informed decisions about their physical and mental well-being.
  • Maintain accurate and up-to-date records of patient interactions, progress, and outcomes.

Requirements

  • Bachelor's degree in human services or related field, or equivalent experience.
  • At least 1 year of experience as a care coordinator in a behavioral health setting.
  • Strong communication, organizational, and problem-solving skills.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Possession of a valid driver's license and access to an insured vehicle.
