Nurse Care Coordinator Opportunity

We are seeking a dedicated and empathetic Nurse Care Coordinator to join our team at GI Alliance. In this role, you will play a vital part in ensuring seamless patient care and excellent outcomes.

About the Job:

  • Coordinate patient care activities, including scheduling appointments and follow-up treatments.
  • Communicate effectively with patients, families, and healthcare teams to ensure comprehensive care.
  • Document patient interactions and interventions, maintaining accurate and up-to-date records.
  • Collaborate with healthcare professionals to develop and implement individualized care plans.

Requirements:

  • RN degree from an accredited school of nursing; BSN preferred.
  • Minimum 3 years professional nursing experience, with at least 1 year in a similar role.
  • Current state registered nurse license and CPR certificate required.

Salary:$60,000 - $75,000 per year, depending on experience.
