Job Summary:

The LTSS Service Coordinator - RN is responsible for managing the overall care of our members with complex needs, including those with chronic illnesses, co-morbidities, and disabilities. You will work closely with our members, their caregivers, and healthcare providers to develop and implement personalized care plans that meet their unique needs and goals.

Key Responsibilities:
  • Conduct thorough assessments of members' needs and develop comprehensive care plans that address their physical, behavioral, and social health needs.
  • Collaborate with healthcare providers, caregivers, and other stakeholders to ensure seamless care coordination and communication.
  • Identify members who are at high risk for complications and develop strategies to mitigate those risks.
  • Manage members with chronic illnesses, co-morbidities, and disabilities to ensure cost-effective and efficient utilization of health benefits.
  • Establish and maintain relationships with members, caregivers, and healthcare providers to ensure ongoing care and support.
Requirements:
  • Current, unrestricted RN license in applicable state(s) required.
  • Minimum of 3 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
Preferred Qualifications:
  • Bilingual (English/Spanish) is preferred.
  • Experience working with older adults in care management, provider, or other capacity, highly preferred.
  • Experience managing a community and/or facility-based care management case load, highly preferred.
