Job Title: Home Health Physical Therapist

LHC Group is seeking a skilled Home Health Physical Therapist to join our team in Kenmore, Washington.

Job Summary

The Home Health Physical Therapist will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment and evaluation, the Physical Therapist will work to determine a treatment plan, perform interventions aimed at improving and enhancing the patient's well-being, and evaluate the patient's progress.

Responsibilities
  • Evaluate patient's functional status, status of all body systems as required for CMS documentation, and physical therapy needs. Consult with physician in development of therapy plan of care.
  • Provide services within scope of practice as defined by state laws governing practice of physical therapy, in accordance with plan of care, and in coordination with other members of healthcare team.
  • Observe and report patient's response to treatment and any changes in condition to nurse supervisor and/or physician.
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice and evidence-based techniques.
  • Instruct and inform participating members of healthcare team, patient, and family/caregivers regarding plan of care, functional limitations, and progress toward goals.
  • Visit patients within entire service area of respective agency and other nearby LHC agencies as needed.
  • Complete progress notes for each visit and submit progress notes per LHC policy.
  • Complete patient evaluations and develop PT plan of care within state-specific guidelines. Report outcomes of evaluation, goals, and anticipated projected frequency of care to nurse supervisor within 24 hours of evaluation.
  • Participate in staff conferences and committees as requested by agency.
  • May supervise home health aides per LHC policy if allowed per state regulation.
  • Provide supervision of PTA as per LHC policy and state regulation.
  • Participate in weekend rotation to provide services for respective agency and other nearby LHC agencies as needed.
  • Prepare written discharge summary and/or written summary report on patient's condition at least every 60 days.
  • Provide in-service education as requested by agency leadership.
  • Adhere to LHC Therapy Standards of Practice in provision of services to all patients.
  • Access education to maintain and enhance clinical knowledge and skills that are up to date with current standards of care for profession and to maintain state-issued licensure.
  • All other duties as assigned.
Requirements
  • Current Physical Therapy licensure in state of WA
  • Current CPR certification
  • Must have current driver's license and vehicle insurance, and access to dependable vehicle
