About the Role:
  • Evaluate patient functional status and physical therapy needs
  • Develop and implement treatment plans to improve patient well-being
  • Provide services within the scope of practice as defined by state laws
  • Collaborate with healthcare teams to ensure comprehensive care
  • Document patient progress and outcomes
Responsibilities:
  1. Evaluate patient's functional status, body systems, and physical therapy needs
  2. Consult with physicians to develop therapy plans of care
  3. Provide services within the scope of practice, in accordance with plans of care, and in coordination with healthcare teams
  4. Observe and report patient responses to treatment and changes in condition
  5. Treat patients to relieve pain, develop and restore function, and maintain maximum functioning
  6. Instruct and inform healthcare teams, patients, and caregivers regarding plans of care, functional limitations, and progress
  7. Visit patients within the service area and other nearby LHC agencies as needed
  8. Complete progress notes and submit them per LHC policy
  9. Participate in staff conferences and committees as requested
  10. May supervise home health aides and provide supervision of PTAs per LHC policy and state regulation
  11. Participate in weekend rotations to provide services for respective agencies and other nearby LHC agencies as needed
  12. Prepare written discharge summaries and/or written summary reports on patient conditions at least every 60 days
  13. Provide in-service education as requested by agency leadership
  14. Adhere to LHC Therapy Standards of Practice in provision of services to all patients
  15. Access education to maintain and enhance clinical knowledge and skills
Requirements:
  • Current Physical Therapy licensure in the state of WA
  • Current CPR certification
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle
