About the Role:- Evaluate patient functional status and physical therapy needs
- Develop and implement treatment plans to improve patient well-being
- Provide services within the scope of practice as defined by state laws
- Collaborate with healthcare teams to ensure comprehensive care
- Document patient progress and outcomes
- Evaluate patient's functional status, body systems, and physical therapy needs
- Consult with physicians to develop therapy plans of care
- Provide services within the scope of practice, in accordance with plans of care, and in coordination with healthcare teams
- Observe and report patient responses to treatment and changes in condition
- Treat patients to relieve pain, develop and restore function, and maintain maximum functioning
- Instruct and inform healthcare teams, patients, and caregivers regarding plans of care, functional limitations, and progress
- Visit patients within the service area and other nearby LHC agencies as needed
- Complete progress notes and submit them per LHC policy
- Participate in staff conferences and committees as requested
- May supervise home health aides and provide supervision of PTAs per LHC policy and state regulation
- Participate in weekend rotations to provide services for respective agencies and other nearby LHC agencies as needed
- Prepare written discharge summaries and/or written summary reports on patient conditions at least every 60 days
- Provide in-service education as requested by agency leadership
- Adhere to LHC Therapy Standards of Practice in provision of services to all patients
- Access education to maintain and enhance clinical knowledge and skills
- Current Physical Therapy licensure in the state of WA
- Current CPR certification
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle
