PACE Center Director Job Description

The PACE Place is seeking a highly skilled and experienced PACE Center Director to oversee the daily operations of our PACE Center. As a key member of our leadership team, you will be responsible for ensuring the delivery of high-quality care and services to our participants.

Key Responsibilities:
  • Oversee the daily coordination and implementation of the PACE Partners Model of Care.
  • Continuously evaluate the participants' changing needs and make necessary program adjustments.
  • Collaborate with center teams to determine best practices and approaches for the PACE Partners.
  • Coordinate home health, rehabilitative, long-term care, and medical/specialty services according to participant care plans.
  • Assist in providing in-service education for the PACE Partners staff.
  • Direct, supervise, and evaluate the performance of the Clinical Manager, Day Center Supervisor, Physical Therapist, Occupational Therapist, Speech Therapist, Recreational Coordinator, Social Worker, Dietitian, and Receptionist.
  • Ensure continuity of operations.
  • Ensure that the job responsibilities, authorities, and accountability of all direct reports are defined and understood.
  • In conjunction with the Executive Director, responsible for maintaining compliance with all state and federal program requirements, obtaining, and maintaining certification under Centers for Medicare and Medicaid Services (CMS) as well as any other state or federal law or rules.
  • Assist in the development and implementation of the organization's PACE Partners Strategic Plan.
  • Assist in program development and preparation of PACE Partner's short-range and long-term plans, budgets and related information systems based on broad organization goals and growth objectives.
  • Participate in all interdisciplinary team meetings and often serve as facilitator.
  • Coordinate the interdisciplinary team's care management process to ensure best decisions are made regarding the participant's changing condition.
  • Assist, as necessary, in the development and adherence of the annual operational and capital budgets for departments of accountability and managing services to meet financial benchmarks.
  • Assist Human Resources Manager as needed in the PACE Partner's operational policies, procedures, protocols, and regulations for direct participant care and support services.
  • Assist and collaborate with Human Resources Manager on employee performance, employee relations, investigations, discipline, and other staff needs.
  • Monitor operations to ensure consistent compliance with established quality assurance standards.
  • Assist Executive Director, as needed, with evaluation of PACE Partners performance against national results and established service standards, budget, and other financial criteria.
  • Perform recruiting duties and collaborate with Human Resources and Recruiting partners on staffing needs for the PACE Partners.
  • Recommend changes to Human Resources Manager and Recruiting in service delivery and staffing as required due to the development and growth of the organization.
  • Participate regularly in all quality assurance/quality improvement committees as part of the overall quality improvement program.
  • In conjunction with the PACE Partners Management team, maintains the stability and reputation of PACE Partners by ensuring that all activities and operations are performed in compliance with local, state, and federal contracts, regulations, laws, accreditation, protocol, licensing, and certification requirements governing operations.
  • Establishes positive working relationships with health and social service providers.
  • Monitor operational compliance with Occupational Safety and Health Administration (OSHA) regulations and agency policies and procedures.
  • Protect privacy and maintains confidentiality of all company procedures, results, and information about employees, participants, clients, or families.
  • Completes all required compliance and continuing education classes and/or training programs and events.
  • Comply with all state and/or federal regulations to ensure the documenting, tracking and monitoring of ordered, care planned or approved DME, home delivered meals, and supplements.
  • Comply with all state and/or federal regulations to ensure the organization is performing assessments, collecting and analyzing the data from these assessments to take actions to improve the quality of the participant's life.
  • Monitoring and oversight of the IDT to ensure unscheduled reassessments by the IDT are being conducted as required per all state and/or federal regulations.
  • Comply with all state and/or federal regulations to ensure the IDT coordinates 24-hour care delivery that meets the needs of each participant across all care settings.
  • Comply with all state and/or federal regulations to ensure that all ordered, approved, or care planned services are provided to the participant.
  • Comply with all state and/or federal regulations to ensure all complaints regarding quality of care or service delivery are properly processed as a grievance.
  • Comply with all state and/or federal regulations to ensure all individual issues within a grievance are reviewed, considered, and resolved.
  • Comply with all state and/or federal regulations to ensure immediately approved SDRs have notification of the approval during the same encounter and/or interaction.
  • Comply with all state and/or federal regulations to ensure all participant medical records are complete, accurate and accessible to all staff.
  • Maintain professional affiliations and any required certifications.
  • Provide administrative on-call coverage on a rotation basis.
Qualifications and Requirements:
  • A minimum of a bachelor's degree in health care services, health care administration, social services, or a related field, RN preferred.
  • Three years of staff supervisory experience in a health care services setting, and a minimum of one-year of experience in providing services to persons having dementia.
  • A registered nurse licensed in Florida, with at least three years of staff supervisory experience in a health care services setting, and have a minimum of one- year of experience in providing services to persons having dementia.
  • PACE experience preferred.
  • Have 1 year of experience working with a frail or elderly population OR, if the individual has less than 1 year of experience but meets all other job requirements, the candidate must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
  • Meet a standardized set of competencies for the specific position description established by the PACE organization before working independently.
  • Either training, or related experience in direction of a health care program.
  • Adult Day Center Supervisory or Nursing home administrator experience preferred.
  • Knowledge of the physical, mental, and social needs of the elderly.
  • Valid Florida Driver's License, required.
  • Active Auto Insurance coverage, required.
  • ACHA Level II background, required.
  • BLS Certification required.
Skills and Knowledge:
  • Working knowledge of the PACE Partners regulations and all other regulatory requirements, highly desired.
  • Working knowledge of the state's healthcare system, local health care, and aging networks.
  • Demonstrated effective leadership and decision-making skills.
  • Effective supervisory skills.
  • Ability to foster collaborative working relationships.
  • Strong group facilitation skills.
  • Working knowledge of physical, mental, and social needs of the frail elderly and their families.
  • Working knowledge of quality improvement and care management systems.
  • Ability to apply creative critical thinking skills to complex issues.
  • Ability to plan, prioritize and follow-up on delegated responsibilities effectively and efficiently.
  • Grasp of the knowledge base from which various disciplines assess the needs of the elderly population.
  • Effective written and oral communication skills.
  • Working knowledge of Microsoft Office Suite (including Word, Excel, PowerPoint, and Outlook) and Electronic Health Records.
