Thrive Skilled Pediatric Care LLC is seeking a dedicated and compassionate Registered Nurse (RN) or Licensed Vocational Nurse (LVN) to provide high-quality clinical home care to medically fragile children in the Pearland, TX area.
Responsibilities:
- Provide skilled nursing care to pediatric patients in their homes, promoting their physical, emotional, and developmental well-being.
- Develop and implement individualized care plans in collaboration with the patient's healthcare team.
- Conduct thorough assessments, monitor patient progress, and report any changes or concerns to the patient's physician.
- Administer medications, perform treatments, and provide education to patients and their families on disease management and self-care techniques.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
Requirements:
- RN/LVN with a current valid license to practice as issued by the State Board of Nursing.
- Current CPR certification.
