Overview:
We are seeking a dedicated Nurse Practitioner or Physician Assistant to deliver comprehensive care for patients with Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD) across multiple settings, including hospitals, dialysis centers, and outpatient clinics. This role involves engaging with a diverse patient demographic, often facing resource limitations. Responsibilities include, but are not limited to:
  • Conducting assessments and managing treatment plans.
  • Ordering, performing, supervising, and interpreting diagnostic and laboratory tests.
  • Formulating diagnoses and initiating treatment protocols, including medication prescriptions in collaboration with Nephrologists.
  • Coordinating care with patients, families, and communities, providing education and counseling.
  • Offering expertise and support to nursing and medical staff to ensure optimal patient outcomes through evidence-based practices.

Qualifications:
To excel in this role, candidates must meet the following criteria:
  • Nurse Practitioner: Completion of a graduate program from an accredited institution, national board certification, and a valid, unrestricted nursing license.
  • Physician Assistant: Graduation from an accredited Physician Assistant program and successful completion of the national certification examination.
  • Current Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) certifications.
  • Clear criminal background check.
  • Commitment to ongoing education and professional development.
  • Valid driver’s license and reliable transportation for travel between various locations.
  • Proficiency in Microsoft Office and familiarity with Electronic Health Records.

  • Effective communication and interpersonal skills to engage with diverse populations.
  • Capacity to manage multiple tasks and prioritize effectively.
  • Flexibility in scheduling to meet organizational needs.

Core Responsibilities:
  • Provide direct clinical care to adult and geriatric patients with CKD and ESRD.
  • Engage in collaborative practice with healthcare teams to enhance patient care.
  • Participate in quality improvement initiatives and adhere to safety protocols.
  • Maintain professional development through continuing education and involvement in professional organizations.
  • Travel to various sites as required to fulfill patient care responsibilities.

Physical Requirements:
  • Ability to travel between sites.
  • Effective communication skills, including hearing and visual acuity.
  • Capability to perform fine motor skills and maintain a stationary position for extended periods.
  • Ability to assist in patient transfers using appropriate techniques.

Work Environment:
The role is primarily conducted in clinical settings, including offices and medical facilities, with moderate noise levels and potential exposure to health risks.
