Key Responsibilities:
- Conduct comprehensive assessments of patients and develop individualized care plans to address their unique needs and goals.
- Provide ongoing care and support to patients, including medication management, wound care, and education on disease management and prevention.
- Collaborate with interdisciplinary teams, including physicians, nurses, and social workers, to ensure seamless care coordination and communication.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Stay up-to-date on the latest clinical research, guidelines, and best practices in geriatric medicine.
Requirements:
- Master's degree in Nursing with a focus on Nurse Practitioner education.
