- Maintain accurate and confidential patient records in both written and electronic formats.
- Engage in ongoing professional development and fulfill annual continuing education requirements.
- Serve as a mentor and consultant within the healthcare community.
- Address complex medical issues while prioritizing quality of life and functional capabilities.
- Respect and uphold patient rights and cultural considerations.
- In collaboration with the medical director and family, assess when it is no longer safe for a patient to live independently and discuss alternative living options.
- Provide guidance on end-of-life care to patients and families.
- Adhere to all legal, regulatory, and accreditation standards.
- Follow organizational policies regarding payroll, billing, and documentation.
- Implement infection control measures to safeguard patients and staff.
- Utilize personal protective equipment (PPE) appropriately, especially during shortages.
- Stay informed about current regulations affecting patient care.
- Valid state licensure as a Nurse Practitioner in SC.
- Certification from a recognized national certifying body as a Nurse Practitioner.
- Current health certificate, physical examination, and TB screening.
- Strong assessment capabilities.
- Excellent verbal and written communication skills.
- Preferred experience in primary care settings.
