St. Leonard is offering a remarkable opportunity for a Full-Time Social Services Assistant.

Our expansive 240-acre campus stands as one of the largest Continuing Care Retirement Communities in the nation, featuring a range of services including independent living, assisted living, memory care, skilled nursing, rehabilitation, and adult day care, alongside a community wellness center equipped with a fitness facility and dining options.

As part of an interdisciplinary team, the Social Services Assistant plays a crucial role in assessing and enhancing the social and emotional well-being of residents and their families. This position is dedicated to supporting adults and seniors, aiming to uphold the dignity, growth, and autonomy of each resident.

Key Responsibilities:

  • Conduct pre-admission interviews with potential residents and their families, detailing all available services.
  • Collaborate with the Director of Nursing to assess patients from hospitals or homes prior to their transfer.
  • Thoroughly explain and document all necessary admission forms and agreements.
  • Oversee the admission process, interviewing new residents and their families to gather information on interests, abilities, and background.
  • Design and implement ongoing training for staff to enhance understanding of social aspects in Long Term Care.
  • Establish and maintain connections with external agencies.
  • Assess resident and family needs in relation to available resources, making appropriate referrals.
  • Coordinate continuity of care services for residents upon discharge.
  • Provide emotional support to residents and their families.
  • Regularly evaluate residents to monitor their progress.
  • Maintain comprehensive social records and discharge plans for all residents.
  • Participate in resident care conferences and contribute to the development and updates of care plans.
  • Engage with community groups as relevant.
  • Attend seminars, workshops, and mandatory in-services as directed by management.
  • Participate in admissions committee meetings and other departmental discussions.

Qualifications:

Education: High school diploma or equivalent; college experience preferred.

Experience: Prior experience in Social Services; experience with older adults is preferred.
