About the Role
We are seeking an experienced Registered Nurse (RN) to join our team as an Enrollment Nurse Coordinator in the Enrollments department. This is a full-time, fully onsite position based in San Diego, California, offering a competitive salary of $92,000 - $105,000 per year, depending on experience, plus a comprehensive benefits package, including health, dental, vision, life insurance, and education reimbursement.

The successful candidate will be responsible for assessing patients' medical records, conducting comprehensive assessments, and making recommendations for admission to our PACE program. The ideal candidate will have a strong nursing background, excellent communication skills, and the ability to work independently in a fast-paced environment.

Key Responsibilities:
* Conduct medical, homecare, and lifestyle assessments for potential St. Paul's PACE enrollees
* Review all medical records and document medications to ensure patients meet level of care requirements
* Document assessments in DHCS tool and EMR
* Manage submissions and notifications regarding required level of care and eligibility for PACE
* Participate in weekly Enrollment Committee meetings to discuss assessment findings
* Complete preliminary home care and clinic care plans for the interdisciplinary team
* Present information on health-related topics to the community
* Provide clinical support in PACE clinics as needed
