Care Coordinator

We are seeking a skilled Care Coordinator to join our team at Spaulding Hospital - Boston and Cambridge. As a key member of our healthcare team, you will play a vital role in ensuring the seamless coordination of patient care from admission through discharge.
Key Responsibilities- Assessment and Planning: Conduct thorough assessments of patients' health and psychosocial needs, developing collaborative care plans that maximize healthcare responses, quality, and cost-effective outcomes.
- Communication and Coordination: Foster effective communication and coordination between healthcare team members, payers, and community resources to minimize fragmentation of services and promote optimal outcomes.
- Patient-Centered Care: Empower patients and families to take an active role in their care, providing education and support to ensure informed decision-making and timely transitions to the next level of care.
- Discharge Planning: Develop and implement discharge plans that meet patients' needs and financial requirements, ensuring a safe and smooth transition to the next most appropriate level of care.
- Cultural Competence: Provide culturally sensitive care that acknowledges and respects patients' diverse backgrounds and needs.
- Documentation and Compliance: Maintain accurate and timely documentation, adhering to department standards and regulatory requirements.
- Education: Bachelor's degree in a related field; Master's degree preferred.
- Experience: 3-5 years of experience in case management, utilization review, or discharge planning in an acute, rehabilitation, skilled nursing, or outpatient setting.
- Licensure: Current, unrestricted license to practice in Massachusetts in a health or human services discipline.
- Communication and Interpersonal Skills: Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Computer Proficiency: Proficiency in Microsoft Office applications and ability to learn new software.
- Certification: Certification in case management preferred.
