Job Summary

We are seeking a skilled Care Coordinator to join our team at Spaulding Hospital - Boston and Cambridge. As a key member of our healthcare team, you will play a vital role in ensuring the seamless coordination of patient care, from admission to discharge.

Key Responsibilities
  • Assessment and Planning: Conduct thorough assessments of patients' health and psychosocial needs, developing collaborative care plans that maximize healthcare responses and promote quality, cost-effective outcomes.
  • Communication and Coordination: Foster effective communication and coordination between healthcare team members, patients, families, and third-party payers to minimize fragmentation of services and optimize outcomes.
  • Discharge Planning: Develop and implement discharge plans that meet patients' needs and financial requirements, ensuring a safe transition to the next level of care.
  • Cultural Competence: Provide culturally sensitive care, incorporating patients' cultural needs into the development of discharge plans.
  • Education and Support: Educate patients and families about treatment options, community resources, and insurance benefits, empowering them to make informed decisions and promote patient self-determination.
  • Documentation and Compliance: Maintain accurate and timely documentation, adhering to department standards and third-party coverage guidelines.
Qualifications and Experience
  • Education: Bachelor's degree in a related field; Master's degree preferred.
  • Experience: 3-5 years of related experience, with a focus on case management, utilization review, or discharge planning in an acute, rehabilitation, skilled nursing, or outpatient setting.
  • Licensure: Current, unrestricted license to practice in Massachusetts in a health or human services discipline.
Skills and Abilities
  • Knowledge of Community Services: Familiarity with community services and healthcare systems.
  • Computer Proficiency: Proficiency in Microsoft Office applications and ability to learn new software.
  • Certification: Certification in Case Management preferred.
