Requirements:
• Current state license as a LPN or MSW (SOCIAL WORKER PREFERRED).
• Minimum of 5 years of experience in a healthcare setting, with at least 3 years in Hospice and/or Palliative Care.
• Reliable transportation and ability to sit, stand, bend, move intermittently, and lift at least 25lbs.
