Southern New England Healthcare Organization, Inc. is seeking a highly skilled Administrative Assistant to join our team in Windsor, CT. This role will provide administrative and operational support to population health initiatives aimed at improving overall health outcomes for patients.

Responsibilities:

  • \
  • Coordinate meetings, events, and training sessions related to population health management.\
  • Assist in updating and maintaining program documentation, including policies, procedures, and guidelines.\
  • Process incoming referrals from member providers and stakeholders, assigning to appropriate team members.\
  • Triage incoming calls for coordination, routing to appropriate team members and ensuring timely follow-up while maintaining excellent customer service.\
  • Update population health program resources and documents, ensuring they are accurate and available. Archive outdated documents.\
  • Demonstrate autonomy, being resourceful in tracking down information to get answers, and clearly defining problems when reviewing with leadership.\
  • Provide administrative support to the Population Health team, including scheduling, correspondence, document management, and other tasks as needed.\
  • Collect, organize, and manage health-related data from various sources.\
  • Assist in analyzing data to identify trends, gaps, and opportunities for improvement.\
  • Assist in preparing reports and presentations based on data analysis for stakeholders.\
  • Utilize multiple software programs, including care management platforms, electronic medical records (EMR), Microsoft Office applications, and payor databases.\
  • Support the development and dissemination of communication materials, such as newsletters, brochures, and educational content.\
  • Assist in coordinating outreach efforts to engage community members, healthcare providers, and other stakeholders.\
  • Respond to inquiries and provide information about population health programs to internal and external audiences.\
  • Participate in quality assurance activities, including audits and evaluations of program effectiveness.\
  • Strictly observe HIPAA regulations and organizational policies and procedures regarding confidentiality of patient information.\

    • Qualifications and Competencies:\
      • \
      • Associates degree preferred with minimum 3 years' experience in a health-related field.\
      • Minimum 2 years of successful operational support experience in healthcare or with a proven track record in a similar role.\
      • Knowledge of medical terminology, medical coding, and/or experience working in a clinical setting.\
      • Knowledge of HIPAA, including Protected Health Information (PHI), and other health-related compliance regulations.\
      • Strong critical thinking, problem-solving, and analytical skills.\
      • Multitasking and time management skills.\
      • Excellent communication and interpersonal skills.\
      • Proficiency in Microsoft Office 365 (Word, Excel, PowerPoint, Outlook) and data management software.\
      • Adaptability and flexibility.\\
      • Physical and Mental Requirements:\

        • Regularly required to use hands to finger, handle or feel objects, tools, or controls; reach with hands and arms; and talk or hear.\
        • Frequently required to maintain a stationary position and move or traverse.\
        • Occasionally required to stand; climb or balance; and stoop, kneel, crouch or crawl.\

        • Vision abilities required by this job include close vision, color vision, peripheral vision, depth perception and ability to adjust focus.\
        • The dexterity necessary to utilize a computer keyboard on a regular basis is essential.\\

Estimated Salary Range: $60,000 - $80,000 per year
