Hospice Physician Opportunity

St. Peter's Health Partners is seeking a part-time Hospice Physician to join its Albany/Rensselaer location. This is an exciting opportunity to collaborate with a respected and compassionate team of caregivers as they continue to provide service to the Capital District.
About the RoleThe Community Hospice is a not-for-profit 501(c)(3) organization that serves eight counties in the Capital Region of upstate New York through three regional offices and is accredited by the Community Health Accreditation Program (CHAP). We are part of St. Peter's Health Partners and work with every hospital in the region to provide hospice services. We also collaborate with 43 skilled nursing facilities and many assistive living/adult homes across our service area.
As a Hospice Physician, you will meet routinely with the entire hospice interdisciplinary group (nurses, social workers, chaplains) for care planning to meet each patient's and family's specific needs. You will serve as a resource to and collaborate with the hospice patient's community medical providers (including prescribing medications), be responsible for certifying the hospice patient's initial and ongoing eligibility for hospice services, help to determine the appropriate level of hospice care, and may serve as the hospice patient's attending physician.
Responsibilities- Visit patients in their homes, nursing homes, and hospitals as necessary
- Routinely participate in the education of medical students, NP and PA students, as well as residents and Geriatrics Fellows
- Share on-call responsibilities with a group of 5 hospice physicians
St. Peter's Health Partners offers a comprehensive salary and compensation package that includes:
- Starting Bonus and Competitive Salary with incentives
- Compensation for 0.5 FTE is between $129,000 and $132,000
