Job Overview:


As a Hospice Licensed Practical Nurse, you will:
· Deliver direct patient care, utilizing a diverse skill set and coordinating necessary services.
· Ensure accurate documentation in accordance with policies, procedures, and regulations.
· Implement the care plan tailored to the patient's diagnosis and condition as developed by the Registered Nurse.
· Collaborate with the Registered Nurse, Patient Care Coordinator, and other personnel to enhance patient care, engaging with families, community agencies, and physicians.
· Accept and accurately transcribe physician orders as required.
· Relay all clinical, progress, and summary notes to the Hospice Registered Nurse for effective care coordination.
· Participate actively in interdisciplinary team meetings.

Qualifications:


· Completion of nursing education from an accredited nursing program.
· Current LPN licensure in the relevant state and a minimum of one year of clinical nursing experience, ideally in an acute care medical/surgical environment.
· Ability to independently manage workload and deliver effective healthcare services to hospice patients.
· Capacity to ensure proper patient care under the supervision of the Hospice Registered Nurse, with thorough, accurate, and timely documentation.
· Strong communication skills, both verbal and written.
· CPR certification required.
