We are seeking a highly skilled LVN - Coordination of Care professional to join our team at Kindred at Home. As a key member of our care coordination team, you will play a vital role in ensuring the seamless delivery of high-quality care to our patients.
Key Responsibilities- Maintain accurate and up-to-date Coordination of Care notebooks and Home Health referral lists.
- Update and report referral issues to the CBA Director/DON or Director of Client Care on a regular basis.
- Prepare and present written reports to relevant staff following case conference meetings.
- Communicate effectively with home health case managers and other stakeholders to ensure timely and efficient care delivery.
- Provide backup support for Special Attendant orientations and make field visits as needed.
- Attend all scheduled meetings and take call on a rotation basis.
- Sufficient education to attain LVN licensure.
- Home Health care experience preferred.
- Basic nursing skills, including evaluation and documentation, are essential.
- Excellent communication and organizational skills are required.
- CBA and PHC knowledge and understanding are preferred.
- Computer knowledge and bilingual skills (Spanish) are also preferred.
- Active LVN license.
