POSITION SUMMARY:
The role involves delivering direct support services to individuals living in community-based environments. The primary focus is to safeguard the health and safety of all clients while fostering independence in various aspects of daily living. Responsibilities encompass community integration, health and wellness promotion, skill enhancement, and successful achievement of personal goals.

KEY RESPONSIBILITIES:
- Offer supervision, assistance, and support to clients receiving services from Almadan, Inc., adhering to their individual support plans.
- Ensure the health and safety of all clients receiving services.
- Provide physical assistance to clients as necessary to encourage independence in daily living activities, including personal care, community engagement, meal preparation, financial management, and social skills.
- Complete all required daily documentation accurately.
- Conduct daily housekeeping tasks and maintain cleanliness of the residence, including laundry, sweeping, mopping, dishwashing, and snow removal; engage clients in these activities when appropriate.
- Provide transportation to clients in line with agency policies.
- Maintain current certifications and training in CPR, First Aid, and MAP, along with any required site-specific training.
- Execute job responsibilities in accordance with agency policies and state regulations.
- Attend and engage in mandatory staff meetings as per agency policy.
- Participate in meetings as directed by the direct supervisor.
- Contribute to the development of individual ISP under the guidance of the Program Director/Manager.
- Ensure the implementation of support strategies outlined in individual ISPs.
- Maintain a flexible schedule, adjusting as needed based on the requirements of clients receiving support.
- Perform additional duties as assigned.
- Consistently uphold the principles of Almadan as outlined in the agency's code of conduct.

MINIMUM REQUIREMENTS: High School Diploma. A minimum of 1 year of experience working with DMH/DDS individual populations is preferred. Must be at least 18 years of age. A valid driver's license and reliable vehicle are required to remain on site.

MENTAL/PHYSICAL DEMANDS: Must be capable of bending, lifting, stooping, and reaching as necessary. Crisis Management/De-escalation training will be provided.
