Dialysis Nurse

We are seeking a highly skilled and experienced Dialysis Registered Nurse to join our team at Rendevor Dialysis. As a key member of our dialysis unit, you will be responsible for providing high-quality patient care, overseeing the complete operation and performance of dialysis treatment, and ensuring compliance with state and federal regulations.
Key Responsibilities- Provide comprehensive assessments of patients' medical, behavioral, pharmaceutical, and social needs
- Develop and implement individualized care plans to address identified needs and improve patient outcomes
- Coordinate care by serving as a patient advocate and resource for patients, families, and healthcare providers
- Facilitate continuity of care across settings, including home, hospital, skilled nursing facility, and acute care facility
- Manage patients during periods of transition to minimize avoidable readmissions
- Assess patients' knowledge of their renal condition and provide education and self-management support
- Provide ongoing reassessment and follow-up to improve patient outcomes
- Develop and monitor quality control measures to ensure continuous quality of patient care
- Participate in performance improvement projects and quality assurance meetings
- Graduate of an accredited nursing program (ADN, BSN, or MSN)
- Current state licensure as a Registered Nurse
- Current CPR certification (AHA BLS)
- 18 months of clinical nursing experience in dialysis within the last 24 months

This role exists in the medical unit of a post-acute facility. This role may include exposure to chemicals and blood-borne pathogens. This role routinely involves the use of both medical and standard office equipment.

The employee frequently is required to stand; walk; use hands to handle or feel; and reach with hands and arms.
