**Job Overview**

We are seeking a highly skilled Service Coordinator to join our team at the Alta California Regional Center (ACRC). As a Service Coordinator, you will play a vital role in providing case management and advocacy services to individuals with developmental disabilities.

Salary: $24.63 - $31.43/hr. (dependent on experience and education)

Job Description:

  • Assist clients and their families in acquiring and maintaining assessed supports and services, including transportation, vocational training, day programming, education, mental health services, medical care, and independent living training;
  • Serve as an advocate for the client with community agencies;
  • Consult with and assist vendors with certifications, applications, and referrals;
  • Participate in the development of Individual Program Plans for clients;
  • Monitor these plans and revise them as necessary;
  • Complete all required forms, documentation, and reports in accordance with regulations and ACRC policies and procedures;
  • Guide applicants and their families through the intake process;
  • Rotate officer-of-the-day duties with other Service Coordinators;
  • Assist co-workers with special projects, unique problems, vacation, and sick relief;
  • Provide emergency on-call services when required.

Required Skills and Qualifications:

  • Bachelor's or Master's degree in Social Work or a related field;
  • Excellent written, verbal, and interpersonal communications skills;
  • Strong time management and organization skills;
  • Valid California driver license and automobile liability insurance;
  • One year of professional experience in social work or a related field (preferably in the field of developmental disabilities).
