Medical Care Manager

A leading social services agency in New York City, Tandym Group, is seeking a skilled Licensed Practical Nurse to join their Adolescent Program.
The successful candidate will work under the Nurse Manager to oversee the medical care management of youth in residential and family foster care.
Key Responsibilities:
The Licensed Practical Nurse will:
Conduct home visits and direct interactions to identify health needs of adolescents in care and their caretakers
Establish a medical home that addresses current and future health care needs for adolescents in care
Act as a liaison between multiple providers to ensure comprehensive health services for adolescents in care
Provide emotional, psychological, and spiritual support to clients and families in a compassionate environment
Utilize multidisciplinary team strategies to resolve client problems and needs
Document client care services by charting in patient and department records, ensuring compliance with regulatory agencies
Communicate with clients and the multidisciplinary team to provide client-related health information
Administer medication and monitor the youth's response to prescribed medication
Perform other duties as assigned by the Nurse Manager.
Requirements:
Graduate of an accredited Practical Nursing program
Current American Heart and/or American Red Cross CPR certification
Strong assessment, clinical, and documentation skills
Patient-oriented approach
Excellent interpersonal and communication skills (written and verbal)
Strong attention to detail and organizational skills
Preferred Qualifications:
Bilingual (English and Spanish) skills
