LPN Medication Care Manager

We are seeking a highly skilled LPN Medication Care Manager to join our team at Sunrise Senior Living. As a Medication Care Manager, you will play a vital role in ensuring the highest quality of care for our residents by administering medications and treatments in a safe and organized manner.
Key Responsibilities- Administer medications and treatments to residents in accordance with their individualized care plans.
- Document medication administration and any changes to resident care plans.
- Conduct Service and Health Updates as directed by the Resident Care Director.
- Participate in the development and implementation of Individualized Service Plans (ISPs) and Individualized Care Plans (ICPs).
- Collaborate with the healthcare team to ensure seamless care transitions and effective communication.
- Maintain accurate and up-to-date records of resident medication administration and care plans.
- Identify and report any changes in resident condition or concerns to the Resident Care Director or Wellness Nurse.
- Participate in ongoing education and training to stay current with best practices in medication administration and senior care.
- LPN, LVN, or state/province specific licensed nurse credential.
- CPR and First Aid certification.
- Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.
- Excellent communication and interpersonal skills.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Flexibility to work varied shifts, including evenings, weekends, and holidays.
