Medical Care Manager

A social services agency in New York City is seeking a skilled Licensed Practical Nurse to join their Adolescent Program team.
Working under the Nurse Manager, the successful candidate will be responsible for overseeing the medical care management of youth in residential and family foster care.
Key Responsibilities:
The ideal candidate will:
1. Identify the health needs of adolescents in care and their caretakers through home visits and direct interaction
2. Establish a medical home for adolescents in care to address current and future health care needs
3. Act as a liaison between multiple providers to ensure comprehensive health services for adolescents in care
4. Provide emotional, psychological, and spiritual support to clients and families
5. Document client care services and maintain accurate medical records
6. Communicate effectively with clients and the multidisciplinary team to provide client-related health information
7. Administer medication and monitor the youth's response to prescribed medication
Requirements:
1. Graduate of an accredited Practical Nursing program
2. Current American Heart and/or American Red Cross CPR certification
3. Strong assessment, clinical, and documentation skills
4. Patient-oriented and empathetic approach
5. Excellent communication and interpersonal skills
6. Strong attention to detail and organizational skills
Preferred Qualifications:
Bilingual (English and Spanish) skills are highly desirable
