Key Responsibilities:
As a Clinical Pharmacist, your duties will include but are not limited to:
- Evaluating patient responses to medication therapies and collaborating with healthcare providers to develop tailored drug therapy plans.
- Ordering and interpreting laboratory tests to assess the effectiveness of pharmacological treatments.
- Initiating, adjusting, or discontinuing medications for chronic conditions in conjunction with the medical team.
- Monitoring pharmacotherapy regimens that require adjustments based on evolving pharmacokinetic factors.
- Administering medications in accordance with established protocols and agreements.
- Assessing nutritional needs and prescribing appropriate enteral or parenteral nutrition in inpatient settings.
- Reviewing medical histories to identify and resolve drug-related issues.
- Consulting with multidisciplinary teams to optimize patient treatment strategies.
- Conducting thorough medication reviews during dispensing to ensure safe and effective therapy.
- Providing education and training to clinical staff regarding new medications and best practices in pharmacotherapy.
- Documenting significant medication interactions and maintaining clear communication with prescribers.
- Facilitating drug utilization reviews as necessary to enhance medication safety and efficacy.
Essential Qualifications:
To be considered for this role, candidates must meet the following criteria:
- U.S. Citizenship is required.
- Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
- Active pharmacist licensure in a U.S. state or territory.
- Completion of a clinical pharmacy internship or residency accredited by relevant professional organizations.
- Current, unrestricted license to practice as a clinical pharmacist.
- Experience in direct patient care within an inpatient or outpatient pharmacy setting.
Knowledge and Experience:
Applicants should possess:
- A graduate certificate from an accredited pharmacy residency program or relevant clinical experience.
- A minimum of two years of full-time experience as a pharmacist in a direct patient care role.
