About the Role

This Community Resource Coordinator position plays a vital role in connecting our senior residents with essential services and resources, enabling them to maintain their independence and dignity.

Responsibilities
  • Conduct assessments to identify residents' needs and preferences, developing personalized care plans to support their well-being.
  • Link residents with available services, including home healthcare, meal delivery, and transportation assistance.
  • Facilitate resident activities and events, fostering socialization and community engagement.
  • Communicate effectively with residents, family members, and staff to ensure seamless coordination of care and services.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes.

Requirements and Qualifications

  • Bachelor's degree in Social Work or related field, or equivalent experience.
  • Minimum 2 years of experience in a supportive care setting, preferably in affordable housing.
  • Strong organizational skills, with ability to prioritize tasks and manage multiple projects simultaneously.
  • Excellent communication and interpersonal skills, with ability to work effectively in a fast-paced environment.
  • Proficient in Microsoft Office 365 and able to learn new software applications quickly.

Estimated Salary Range: $25.50 - $30.50 per hour (dependent on experience)
