Job Summary

We are seeking a dedicated Clinical Care Coordinator to join our team at NavitsPartners. As a key member of our healthcare team, you will be responsible for coordinating LPN duties and responsibilities for assigned patients, ensuring seamless care delivery.

Key Responsibilities:

  • Collect and analyze patient data to inform treatment planning and case management.
  • Collaborate with multidisciplinary teams to assess patient needs and develop comprehensive care plans.
  • Provide emotional support and education to patients and their families, promoting independence and empowerment.
  • Administer prescribed medications and monitor patient responses to treatments.
  • Respond to emergency situations and perform first aid and CPR as needed.

We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience and qualifications.
