Job Summary:

The ideal candidate will have experience conducting Annual Wellness Visits and be comfortable with home and telehealth visits. This is a full-time position with a flexible schedule and a fun and engaging work environment.

Responsibilities:
  • Conduct annual wellness visits daily for primary care management of patient caseload
  • Manage both acute and chronic conditions through direct intervention like medication prescription and patient education
  • Consultation with collaborating Physician as appropriate
  • Complete documentation of patient encounters, funding sources, and billing according to policy
Requirements:
  • Current California Nurse Practitioner license
  • Nurse Practitioner Certification from either the ANCC or AANP
  • Current DEA licensure
  • Active CPR Certification
  • 1-2 years of NP experience (home care and geriatrics are a plus)
