We are seeking a skilled and compassionate Nurse Practitioner to join our team at a Santa Clara based clinic. As a Nurse Practitioner, you will play a vital role in improving individual and community health.
Job Summary:The ideal candidate will have experience conducting Annual Wellness Visits and be comfortable with home and telehealth visits. This is a full-time position with a flexible schedule and a fun and engaging work environment.
Responsibilities:- Conduct annual wellness visits daily for primary care management of patient caseload
- Manage both acute and chronic conditions through direct intervention like medication prescription and patient education
- Consultation with collaborating Physician as appropriate
- Complete documentation of patient encounters, funding sources, and billing according to policy
- Current California Nurse Practitioner license
- Nurse Practitioner Certification from either the ANCC or AANP
- Current DEA licensure
- Active CPR Certification
- 1-2 years of NP experience (home care and geriatrics are a plus)
