Job Title: Home Health Physical Therapist

VNA Health Group is seeking a skilled Physical Therapist to join our team in Orange, New Jersey.

Job Summary:

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our home health patients in Newark, Livingston, Millburn, and Short Hills. As a Physical Therapist with VNA Health Group, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities:
  • Assess and evaluate patients for physical therapy needs and develop individualized plans of care.
  • Provide skilled physical therapy services to patients in their homes, including exercises, stretches, and other interventions to improve mobility and function.
  • Collaborate with physicians and other healthcare professionals to ensure comprehensive care.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Physical Therapy licensure in New Jersey required.
  • 2-3 years of prior clinical experience and community-based experience preferred.
  • Strong computer skills and excellent communication and interpersonal skills.
  • Valid New Jersey driver's license.
