Job Summary:The ideal candidate will have a strong background in physical therapy and excellent communication skills. They will be responsible for assessing and evaluating patients, developing and implementing individualized plans of care, and providing ongoing therapy services.
Responsibilities:- Assess and evaluate patients for physical therapy needs and develop individualized plans of care
- Provide skilled physical therapy services to patients in their homes
- Document all changes in patient condition and update care plans accordingly
- Identify counseling needs and plan appropriate teaching strategies
- Monitor quality and appropriateness of services provided
- Adhere to agency infection control and safety policies
- Physical Therapy Licensure in NJ required
- 2-3 years of prior clinical experience and community-based experience preferred
- Strong computer skills
- Strong communication and interpersonal skills
- Valid NJ driver's license
